Prediction of Coronary Angiography Results by Flow Mediated Dilation Assessments with Photoplethysmography

  8. Cardio Vascular Diseases (CVD) is currently the most important cause of mortality in the world. Coronary artery disease (CAD-with 3 major epicardial vessels) is one of the significant CVD. Coronary angiography remains the gold standard for identifying the presence and absence of stenosis due to CAD. It is performed by inserting a catheter into the coronary artery, directly injecting radiopaque contrast material there and recording radiographic images in different view of arteries. Images of this invasive way show severity and area of stenosis.
    A sign of CAD is endothelial dysfunction (inner artery layer) in peripheral arteries like the brachial artery. Due to a significant correlation between brachial and coronary endothelial functions, investigating changes in brachial artery has become a common way for evaluating the status of coronary arteries. Flow-mediated dilation (FMD) in the brachial artery which is measured by B-mode high resolution ultrasound is a common non-invasive way for endothelial function assessment. The results of FMD test depend on the skill of operators and dimension and cost of ultrasonic imaging devices are high so they are two limitations for this test. Therefore recording of PPG signals is proposed as an alternative for ultrasonic imaging.
    The aim of this study is the prediction of angiography results by assessment of PPG signals which are recorded during FMD test. Signals were recorded from two groups. The first group is patients of Tehran Heart Center who underwent coronary angiography (87 data sets), the other one was a group of staffs of Sharif university (11 data sets). 48 data of collected database were used and classified in three groups based on degree of coronary stenosis: 1- Patients with normal coronary arteries or with mild stenosis. 2- Patients who have one major epicardial vessels with ≥70% stenosis, 3- Patients who have two or three major epicardial vessels with ≥70% stenosis. By analyzing PPG signals in the time and time-frequency domains and extracting suitable features 2-class and 3-class classifiers were designed. The results of 2-class analyzing show a 91.8% accuracy for classification of group 1 and 2; 90.8% for group 1 and 3 and 96.8% for groups 2 and 3. The accuracy of classification of all 3 classes was 91.7%. The most suitable features relate to 6.25-20 Hz and 0-0.05 Hz frequency bands. As a result this method is proposed for screening endothelial function
