Safety Assessment for H2S Releasing in Arak Heavy Water Factory (GS(03) Plant) Using Probabilistic Safety Assessment Method (PSA)

  8. In general, release of hazardous gas in chemical process plants could be due to two reasons. First is failures due to degradation of components that have inventory of hazardous gas in standard process conditions, and the second is deviation of system from standard process conditions because of different failure modes of related components, e.g. controller equipments. The purpose of this study is, calculating the probability of H2S releasing in GS(03) plant of Arak Heavy Water factory, using PSA methods and its risk assessment. At First, fifteen groups of initiating events were identified with Hazop study for desired system. Then, for calculating the frequency of each event, the combination of event tree and fault tree was used in SAPHIRE software. In addition, in this thesis, for reliability analysis of similar components with parallel conditions in fault trees calculations, effect of common cause failure (CCF) analysis also was considered. The results of this study shows the frequency equals to 9.622 e-7and uncertainty equals to [6.806 e-8 1.787 e-6] from uncertainty analysis in interval of 5,95 percent for H2S release accident, without considering direct release from components due to degraded failure. Risk assessment in this study was implemented with one of the newest risk matrix presented in [25]. This matrix shows medium safety condition for hazard of release accident in GS(03) plant. Medium safety condition is margin of acceptability. Next, effect of failure of different components, common cause failure (CCF) and human error were ranked using importance analysis. At the end, suggestions were presented to improve the safety of GS(03) plant.
