Experimental Study of the Effect of Bone Density on the Fixation Strength of Graft by Bashti Bone Plug Technique (BBPT) and Comparison with the RCI Interference Screws in Anterior Cruciate Ligament Reconstruction

  8. Bashti Bone Plug Technique (BBPT) is an implant-less method which can be used in an Anterior Cruciate Ligament (ACL) reconstructive surgery. The technique uses a core bone plug to fix the tendon graft into the bone tunnel. This study investigates the effect of the core bone density on fixation strength in BBPT method. A series of experimental test has been performed to examine the hypothesis of the study. Thirty fresh bovine digital extensor tendon used in three different groups. Sawbones blocks with densities of 160, 240 and 320 kg/m^3 used in substitute of the bone to model the effect of density on the strength of the fixation. The core bone plugs and tunnels created using a custom made drill bit from the sawbones blocks. Tendons been doubled to create a doubled fashion tendon graft with overall diameter of 7.5 mm. The graft then passed through the tunnel and the core bone plug was inserted to fix the graft into the tunnel. Samples tested using a universal tensile test machine. A pre-load of 5-20N with frequency of 1.0HZ for 10 cycle was applied for preconditioning of the fixation. Then cyclic load between 20-100N for 100 cycle with frequency of 1.0HZ followed by a single cycle load to failure with a rate of 20mm/s. For group 1 (160kg/m^3) stiffness was 76.7N/mm and maximum load to failure was 172.2N. While for group 2 (240kg/m^3) the stiffness and failure load was 86.6N/mm and 310N. Although, the result in group 3 (320kg/m^3) was 93.9N/mm and 358.2N respectively for the stiffness and failure load. The amount of slippage in group 1 was 3.93mm, in group 2 it was equal to 3.49mm and in group 3 it was equal to 2.02mm. No significant differences was found between groups 2 and 3. Also, 10 test was preformed on RCI screw and Sawbone of group 3 with the same conditions as before. For this test, stiffness was 158.95N/mm and maximum load to failure was 693.18N. The results of this tests showed that Bashti technique need to more tendance after reconstruction process. Finally, the tool was designed and builded to easily block insertion
