  8. According to privileged risks existing in process industries and intensity of their consequences, Safety investigation and risk assessment are highly significant in these unites. Execution of risk assessment in process unites has been constituted from two qualitative and quantitative parts. The most crucial issue in risk assessment procedure is uncertainties related to data and needed information, besides of various models presented. This subject frequently relates to deficiency, ambiguity, and inaccessibility of necessary information for specifying of risk assessment characteristics i.e., event reproducibility and their intensities. In this work for overcome these constraints, bow-tie fuzzy model have been exploited for implementing risk assessment procedure. Analysis of fault-tree and event tree which are the main part of bow-tie model, have been conducted based on fuzzy theory and other researchers’ opinions. Investigation of the main event fault probability takes part the basis of fault-tree analysis, made quantitative by fuzzy logic. In event tree operating parameters have been involved in calculation of possibility of different events occurrence and intensity of consequences by fuzzy deduction system. Eventually, the fuzzy model has been applied on Ethylene oxide purification column in Arak Petrochemical Complex and the results have been presented. The resaults shown that safety of purification column is very high
