- The rapid adoption of the Internet of Things (IoT) has led to significant advancements in healthcare digitization and modernization. However, given the sensitive nature of medical data, security and privacy of such data have always posed major challenges. Moreover, resource constraints of IoT devices have necessitated the use of cloud resources for handling massive amounts of data in traditional approaches, introducing issues like single points of failure, processing delays, and security vulnerabilities. In this context, blockchain technology shows promise for enhancing security, decentralization, and returning data ownership. A robust solution should maintain confidentiality, enable access controls, and ensure data integrity, while also being scalable, cost-effective, integrable with existing systems, and infrastructure. However, current blockchain research has only addressed a subset of these requirements. This paper presents a solution incorporating all the above features along with an real-world implementation and comprehensive analysis. The proposed framework utilizes Proof-of-Authority consensus in the blockchain layer, smart contracts for access control, and advanced cryptographic algorithms for secure communication channels. It addresses common challenges around security and privacy with medical data. A comprehensive analysis and full examination in a simulated environment demonstrate the solution’s security and privacy improvements. Performance results also show low overhead and operational feasibility due to reasonable costs. The simulation validates that the solution enhances medical data security and privacy and as an affordable, practical system
