Studying the Kinematics Compensation Mechanism in the Gait of Individuals with Excessive Anti-version using a Subject-Specific Musculoskeletal Model

- In cases with excessive anteversion even after adolescence, surgeons have to choose osteotomy surgery to modify this abnormality. This surgery and modifying the anatomical problem does not necessarily lead to modify the patients’ gait cycle abnormalities; the authors’ hypothesis is the probable reason may be the persistence of the implement of the patients’ kinematics compensation mechanism even after the surgery. This mechanism is used to almost correct the gait of those with excessive anteversion. First, by using gait analysis text books, a patient’s gait cycle is analyzed; this analysis is consist of 3DGA with EMG and observational gait. Then, by using OpenSim, the patient’s patterns of muscle activations are obtained. The objective of this project is to analyze gait and develop a patient-specific model in order to study the kinematics compensation mechanism in the patient.The gait analysis asserts that the gait abnormalities mostly are a direct consequence of anteversion; these imperfections do not cause major distinctions in the muscle activity relative to normal. The muscle activity also proves that consideration of the excessive anteversion in defining the knee axis leads to match better with EMG signals.Among gait abnormalities, excessive hip abduction and adduction in swing and decrease in the range of hip rotation are the patient’s kinematics compensation mechanism. The muscle activity also confirms this; exceeding activity of internal and external rotators lead to decrease the range of hip rotation and the adductors’ and performis’ activation are the cause of the excessive hip adduction and abduction. These imperfections are used to avoid the two legs encounter during the gait and also the gait becomes much more normal
