Simulating the Provision of Services to the Injured at Transfer Points and Providing a Forecast of the Number of Resources Needed

- In mass casualty incidents such as earthquakes, hospitals and emergency response systems face significant challenges. To manage this critical situation, transfer points have been introduced as a strategic solution, enhancing the speed and efficiency of casualty transfers, thereby improving the overall performance of hospital services. The aim of this study is to develop a discrete-event simulation model that simulates and evaluates the provision of services to casualties at transfer points while predicting the optimal number of resources needed, including ambulances, triage staff, and medical personnel, in various emergency conditions. Additionally, a location model is designed to optimally select the locations of transfer points and field hospitals, the results of which serve as input for the simulation problem. The model is based on the characteristics of Tehran’s District 16, and the key assumptions of this study include casualty triage, outpatient treatment at transfer points, the presence of emergency centers, traffic and route failures, and the efficiency of personnel. This model is employed to balance available resources with emergency demand, considering various crisis parameters. The simulation results in the worst-case scenario (10% of the total population injured) predict the need for 21 triage staff, 81 medical personnel, and 204 ambulances. By simulating different crisis scenarios, this research seeks to provide practical solutions for resource management and improving emergency conditions. The findings can be used to optimize crisis management systems and enhance the effectiveness of emergency response operations, ultimately contributing to reducing casualties during incidents
